Ghrelin rebound describes the sustained elevation of circulating – and the restoration of ghrelin receptor sensitivity – that follows a period of or significant weight loss. It is not a transient post-meal fluctuation but a persistent shift in the body’s hunger baseline that can last weeks to months after active restriction ends.

The mechanism has two compounding components. First, weight loss itself reliably increases circulating ghrelin: as adipose tissue and body mass decline, ghrelin-producing cells in the stomach fundus upregulate output, generating a stronger hunger signal than existed before the weight was lost. Second – and less widely understood – weight loss restores ghrelin receptor sensitivity in the hypothalamic arcuate nucleus. In , / neurons can become partially resistant to ghrelin signalling; following caloric-restriction-induced weight loss, this resistance resolves, meaning the elevated ghrelin signal is now received by a more responsive receptor population. The result is that returning hunger after weight loss is not simply hunger returning to its pre-diet level – it is often a stronger hunger signal acting on a more sensitive system.

This phenomenon applies across the routes by which weight is lost. Caloric restriction diets, very low energy diets, and GLP-1 receptor agonist medications all produce ghrelin rebound upon cessation or dose reduction. In the case of GLP-1 RA discontinuation, the rebound is compounded: the pharmacological suppression of appetite is withdrawn at the same moment that ghrelin – elevated by the weight already lost on medication – reasserts itself without the override. The subjective experience is frequently one of hunger that feels more intense than anything remembered from before treatment, which is physiologically accurate rather than psychological. Dietary strategies that activate the – meals prioritising protein and fat over rapidly absorbed carbohydrate – attenuate the post-meal ghrelin drop delay and sustain GLP-1 and PYY counter-signalling, providing the most accessible nutritional approach to managing ghrelin rebound without pharmacological intervention.
