Catagen is the shortest and structurally most dramatic phase of the hair cycle; a precisely coordinated regression programme that dismantles the transient inferior segment of the whilst preserving its permanent upper portion and the intact for the next cycle. It is not follicle death: the stem cells in the bulge are unaffected, the dermal papilla survives in a condensed state, and the structural capacity to regenerate the inferior segment fully in the next is preserved throughout.

The Catagen Programme

Catagen begins when matrix apoptosis is triggered by the progressive withdrawal of Wnt signalling from the dermal papilla, alongside upregulation of FGF5 in the lower outer root sheath – the molecular signal that tips the anagen-catagen transition. The bulb epithelium involutes upward as cells die, leaving a trailing epithelial strand in its wake. The dermal papilla, rather than being destroyed, is carried upward along this strand, condensing and withdrawing to a resting position directly beneath the bulge region. The club hair – the fully keratinised hair shaft with its characteristic brush-like base – remains anchored in the follicle through the strand structure, held in place until the exogen phase of telogen.

In , the catagen transition is triggered earlier than it should be. DHT drives 2 upregulation and premature Wnt withdrawal in the DP, compressing anagen duration and initiating the regression programme before the follicle has completed a full productive cycle. Over successive shortened cycles, the entire inferior segment regenerates to a progressively shallower depth – the structural basis of terminal-to-vellus miniaturisation.

Clinical Application

Catagen is not directly targeted by any of our treatments at Creative Touch; it is a regulated programme, not a pathological event, and attempting to block catagen entirely would simply arrest the normal cycle. What treatments can do is delay catagen initiation by extending anagen, keeping the DP’s Wnt signalling above the threshold that triggers the regression programme for longer through growth factor support and pathway modulation.

The catagen timeline also has a practical consequence for treatment scheduling: because catagen is only two to three weeks and leaves the follicle infrastructure intact, the window between anagen end and the start of telogen is brief. For clients undergoing treatment series for AGA or post-TE recovery, the catagen phase itself requires no specific management, the focus is on extending the anagen that precedes it and supporting anagen re-initiation in the telogen phase that follows.
